I give my consent for Genesis Support and partner laboratory to carry out a test for the presence of SARS-CoV-2 ( Covid 19) or other variants and for the Test Report and any supporting documents including the information contained in this form to be issued to the Nominated Person (named above) and any other authorised persons or authorities where appropriate. I understand that if the test is positive, my details will be provided to Public Health England and I may be contacted by NHS Test and Trace. I understand that by opting to test for day 2 and day 2 & 8 my details will be provided to Public Health England. I accept responsibility for returning this kit to the laboratory in good time for the test to be carried out prior to any travel arrangements made or for the mandatory testing of SARS-Cov-2 (Covid 19) for International arrivals.
